Здравствуйте, подскажите какой тег использовать для увеличения значения, пример на картинке. Вроде, про такой тег я слышала. Если есть тег прогресс бар, значит и такое должно быть.

Есть форма некая Анкета. В анкете такой вопрос:
Вопрос - Когда вы посещали поликлинику
Ответы - 1) Посещали (указать дату и год) 2) Не посещали
Как сделать условие если пользователь поставил галочку "Посещали" показывалось текстовое поле для ввода года и даты?
